BRING THESE DOCUMENTS BACK TO COURT. MAKE SURE THAT YOU MAKE AND BRING COPIES. THE
COURT KEEPS ALL EVIDENCE SUBMITTED. THE COURT SHALL NOT MAKE COPIES FOR YOU.

PAY STUBS
The last (3) three pay stubs from all your jobs.  If you are now receiving unemployment benefits, worker's compensation,
disability, social security, veteran's benefits, public assistance or any other income, bring proof of how much you receive.

TAX RETURNS
Your tax returns for the previous year.  Bring your Federal and State returns, all attached schedules and W-2 or 1099 form.  If
you filed electronically, bring a printed copy of your return.

FINANCIAL AFFIDAVIT
This is the booklet attached to your court papers and asks you to list your income, assets, expenses and asks for information
about your insurance and licenses. You must complete the entire booklet and have it notarized before coming to court.

BIRTH CERTIFICATE/ACKNOWLEDGMENTS OF PATERNITY
On all Paternity or Child Support petitions parties are required to submit an original and two (2) copies of the Birth Certificate
and if applicable, Acknowledgment of Paternity.  Originals will be returned & copies shall become part of the Court File.  

IN ADDITION, YOU MAY BE REQUIRED TO BRING OTHER DOCUMENTS WHICH INCLUDE

JOB SEARCH
A list of all the places you have looked for a job.  Include the date you went, the name of the employer, the person you spoke
with and the type of job for which you applied.

CHILD CARE/TUITION
(1)Proof of the cost of tuition, scholarship and payment; your child(ren)'s report cards and any recommendation for special
classes or tutoring; (2) proof of the cost of childcare.  Bring proof of payments that you've made for the last three (3) months.  
The childcare provider may be required to testify.
If you have a court order or written agreement for child support, maintenance or alimony for any other child(ren) or ex-spouse
that you want the court consider, bring the order and proof of payment of those orders..
